Assisted living provides the safety and peace of mind of on-site care professionals while allowing seniors to age comfortably in place. At Oak Park Senior Living, we designed this program to give residents the support and care they need to enjoy a higher quality of life while confidently maintaining independence. Our compassionate, professional staff provides needed assistance with healthcare, personal care, and household tasks, as well as planned social and recreational activities.
Our assisted living residents receive professional care from our on-site health and care professionals, including nurses. They also enjoy home-cooked, restaurant-style meals prepared by our professionally trained chef and dining team. Breakfast, lunch, and dinner can be enjoyed in our cozy and gorgeous dining areas, or residents can request in-unit meals ahead of time for an additional fee.
We want our seniors to enjoy a full and happy life at Oak Park. We encourage assisted living residents to partake in our planned activities for socialization and to cultivate personal interests. Our activities calendar makes planning easy, and we fill it with a variety of fun and exciting opportunities like live performances, arts and crafts, community parties, and much more. We also offer educational activities, exercise classes, and group outings/field trips. Whether you want to try something new or an opportunity for a favorite hobby, there’s always something happening at Oak Park Senior Living.

Prior to moving into our facility, our Nursing Director completes a health assessment to determine the most effective and economical service plan. The service plan will define the Assisted Living resident's needs, schedule of services, and financial obligations. Charges to the service plan will be made in consultation with the resident, physician, and/or responsible party. One-time fee applies.
The Home Health Aide (HHA) provides supportive services identified in your service plan. These services may include medication reminders or assistance (when medications are set up by an Oak Park Senior Living Nurse), safety checks, and assistance with activities of daily living. Home Health Aide visits occur in 15-minute increments.
Package A: 2 visits per day, up to 15 hours of services per month
Package B: 4 visits per day, up to 30 hours of services per month
Package C: 6 visits per day, up to 45 hours of services per month
Package D: 8 visits per day, up to 60 hours of services per month
Package E: 10 visits per day, up to 75 hours of services per month
Package F: 12 visits per day, up to 90 hours of services per month
These packages are often appropriate for seniors who only have a few care needs but require assistance with medication compliance. If a resident requires the staff to spend more time than described below, the resident will be charged an additional fee for each 15-minute increment of either HHA or licensed nurse.
Package A: 2 HHA visits per day to remind or assist resident with medication(s). This also includes medication set-up by a licensed nurse.
Package B: 4 HHA visits per day to remind or assist resident with medication(s). This also includes medication set-up by a licensed nurse.
Medication set-up: A nurse will place medications in individualized, labeled containers for each client and will place the containers in his/her apartment. The service includes coordination with the physician and pharmacy for medication orders, compliance assessment, response to medications, and consultation with resident or responsible party as needed.
Medication change: A nurse will adjust the medications to comply with the revised medication order and reset the resident’s labeled containers.
Nurse visit: A minimum of 30 minutes with an additional fee for each 15-minute increment thereafter. The nurse will provide services ordered by the physician and agreed upon in the service plan.
Nurse telephone consultation on evenings, weekends, or holidays: On a per-call basis with physician or family contact.
